The Application

PLEASE READ NEW INSTRUCTIONS BEFORE YOU APPLY. THIS WILL ENSURE THAT WE RECEIVE A COPY OF YOUR APPLICATION.

A) Please print your web application to a PDF. immediately after you submit and email the file to apply@makework.is.

B) If you can not print to PDF on your computer, please email us a .doc or .rtf file with your proposal. Include all responses to the application questions. We also recommend saving a copy of your application responses in a separate document before you apply. Thank you for your patience and understanding.

As you begin this process, please note that this application cannot be completed in phases. The application must be completed in a single session and therefore, it is strongly advised that large bodies of text be drafted in a separate document file on your computer. Please format your responses as Plain Text in Notepad or TextEdit. Your composed responses may then be copied and pasted into this online application for submission.

    Provide an itemized list of expenses (e.g. salary, supplies, equipment, travel, or other expenses). The budget should be as descriptive and specific as possible. Each itemized description is limited to 50 words. The table below will help you plan out each budget expense; the online application form provides up to ten spaces to describe your budget request. A sample budget is available for reference.

    If additional funding is required beyond the MakeWork grant award, describe the additional expenses and the funding sources you have available to cover these costs. This description is limited to 150 words.

  10. Work Samples Description (limited to 100 words)

    Provide a title and brief description for each work sample that you submit. This list should be in the same order as the work samples that you send to apply@makework.is. Applications will not be accepted if work samples are not submitted in the correct order and are not in the correct file format.

    The following extension types are supported: .jpg, .pdf, .doc, .mpg, .mp3, .mov, .m4v, and/or .mp4

  11. Work Samples

    Email 5 work samples to apply@makework.is. There is a 25MB limit on gmail attachments. Please plan accordingly.

    Applicants may use images and/or writing samples. Each file may not exceed 5MB.

    Applicants may use video, music and/or other forms of new media. Such work should not exceed 5 minutes. If you are submitting video work, the videos should be uploaded to vimeo.com with links included in the Work Sample File Description above. Applicants may use audio files. Such work should not exceed 5 minutes. The files will need to be attached in an email to apply@makework.is.

Submission Certification and Waiver of Liability

By clicking ‘SUBMIT’ below, I certify that I meet the eligibility requirements as outlined in the MakeWork Grant Guide and will comply with all award requirements, if awarded. I understand that I am required to attend four, three-hour sustainability sessions, offered through MakeWork, during the grant award period as a condition of the award. I also agree to participate in a self-initiated public showcase of my work and post on a monthly blog, if awarded. I also understand that incomplete applications will not be processed.

Further, I myself and my heirs, executors, administrators, and assigns, hereby release, waive and discharge MakeWork, as well as officers, directors, employees, agents and volunteers of and from any and all claims which I or my heirs, administrators and assigns ever may have against any of the above for, on account of, by reason of or arising in connection with such activities or my participation therein, and hereby waive all such claims, demands, and causes of action.

Further, I expressly agree that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the laws of the State of Tennessee, and that if any portion thereof is held invalid, it is agreed that the balance shall, not withstanding, continue in full legal force and effect. I currently have no known physical or mental condition that would impair my capability for full participation as intended or expected of me.

I have carefully read the foregoing release and identification and understand the contents thereof and sign this release as my own free act.
